Update on the Distribution of the Annual Form 1095-C

Document that provides information about the health insurance coverage offered to you.

Beginning with the 2025 tax year, employers are no longer required to automatically send Form 1095-C to individuals. Instead, the requirement to furnish this form is satisfied if we provide     clear, accessible notice that the form is available upon request.

How to Request Your Form 1095-C

If you would like to receive a copy of your Form 1095-C, you may request one at any time by either of the following methods.

Via Written Letter:

Timeline for Providing the Form

Once we receive your request, we will provide your Form 1095-C the later of January 31, 2026 or 30 days after the date we receive your request.

Your form will be sent to you by your preferred delivery method, such as postal mail or by pick-up directly at the Office of Human Resources.

Please note that while Form 1095-C is not required to file your federal income tax return, it may be useful for your records or to verify health coverage information.
